Opthalmology Referral Service

David Habin BVM&S DVOphthal MRCVS is based at our Mirfield surgery, which is part of the Calder Veterinary Group and from here he provides a comprehensive Ophthalmology referral service, covering all domestic species.

Clinics will be held daily on an appointment only basis although any emergency cases can be catered for. The surgery is fully equipped with improved operating facilities and specialised equipment relating to treatments of a large range of ophthalmological conditions. David would be happy to discuss any ophthalmology cases whether they were to be subsequently referred or not. He will also continue to perform examinations under the BVA/KC/ISDS Eye Scheme.

To make an appointment
Prior to your client contacting us please telephone or fax with case details and presenting problem or brief reason for referral.

You can make an appointment on behalf of your client or you can ask your client to call us direct.

Please give your client an owner information sheet prior to their arrival at our hospital.

Please inform clients that fees are to be paid at the time of examination or if admitted at the time of collection. We are quite happy to claim direct from your clients insurance company but we do need proof of insurance.



Referral Letter

Please fax or post this to us. A referral letter should provide a summary of the history and medical findings. Please also specify a reason for referral eg, for consultation or for scan only etc. Please ensure that all X-Rays, ECG's, lab work etc are brought to us with your client or sent to us in advance of the consultation. Production of these may avoid unnecessary repetition of work.
